When you contact us for a
New Claim appointment you will be advised of what you will need
to provide to us at your appointment and to complete you
claim.
Accepted Proof/Evidence checklist
We must see the latest original documents. We
cannot accept photocopies. We need the same evidence of income for
you, your partner (if you have one) and any other adults living as
part of the household. We will check and take copies of the
documents:
Proof of Identification
We cannot process your claim unless we have identification for
you and your partner. Please provide proof of such as driving
licence, a valid passport, medical card, official letters from
Department of Works and Pension or HM Revenue and Customs, birth
certificate etc.
Proof of National Insurance Numbers
We cannot process your claim unless you give us evidence of you
and your partners' National Insurance Numbers; such as, National
Insurance Card, payslips, letters from Department of Works and
Pension or HM Revenue and Customs.
Proof of Capital, Savings and Investments
Please provide proof of all your bank, building society or post
office accounts (full statements), Premium Bonds, National Savings
Certificates, ISA's, stocks, shares and unit trusts. The evidence
you send, must show details for at least the last two months. We
need to see evidence of any interest or dividends you get on
investments and savings. If you have ownership or part ownership in
any properties other than the one you live in we will need to
know.
Proof of Earnings
We need your last five payslips if you are paid weekly, your
last three payslips if you are paid fortnightly or your last two
payslips if you are paid monthly. If you do not have the sufficient
payslips then please ask your employer to fill in the
Certificate of Earnings form.
If you or your partners are
Self-employed, we will need to see records and accounts, you
will need to contact
us for further advice.
Evidence of other income
Please provide original award notice letters
for student grants and loans. We also need proof of any other
income you receive, including rent from boarders
Evidence of Benefits, Allowances and Pensions
We may be able to check direct with a Benefits Agency on certain
benefits you receive.
Any private pensions you receive we will need to see slips or
letters or original bank statements (of amounts
paid straight into your account).
Evidence of private rent and tenancy details
You and your landlord will need to complete a
Certificate of Rent or you can provide your tenancy agreement
or contract, but the proof must show:
- The landlords full name and address
- If your Landlord has an appointed Agent, then we will require
their name and business address
- The dates when the tenancy began and when you moved in (if
different) and
- The services, facilities and charges included within
rent.
Evidence of money paid out
Please complete a
Child Care Cost form completed by a registered childcare
provider.
For
Second Adult Rebate claims we would still require proof of
Identification and National Insurance for the claimant and proof of
the Second Adults Income.
If you will fail to provide all information required it will
either delay your claim being assessed or prevent your claim from
being processed, therefore no money will be paid out.
